This Notice of Privacy Practices explains how A Better Version Collaborative Counseling may use and disclose your Protected Health Information (PHI) and how you can access your records.   • Amendments – If you believe your health information is incorrect, you may request a correction.
  • Restrictions – You may request restrictions on how we use or disclose your PHI, though we are not always required to agree.
  • Confidential Communications – You can request that we contact you in a specific way (e.g., via mail instead of phone).
  • Accounting of Disclosures – You may request a list of instances where your PHI was shared, except for treatment, payment, and healthcare operations.
  • File a Complaint – If you believe your privacy rights have been violated, you may file a complaint with A Better Version Collaborative Counseling or with the U.S. Department of Health and Human Services (HHS).
